Why Get Involved?
The Oshkosh West High School Athletic Program is an important part of your total educational experience. It provides students with an opportunity to develop physical skills and be part of a team effort. Research tells us that students who participate in athletics are better able to transfer their experience to other activities throughout their lives. Student-athletes earn better grades, have higher attendance rates, and fewer discipline problems than students who don’t participate. Your four years of high school will go by quickly. Make sure you take advantage of your opportunities while you still can.

How to Participate
Participation requirements
The following must be on file with the school before an athlete may start practice:
- his/her physical card and/or athletic permit card
- acknowledgment of receiving the school athletic code and WIAA Rules of Eligibility
- a concussion information sheet signed by both the student-athlete and his or her parent or legal guardian
- a signed agreement of parents and athlete form and insurance waiver form and an athletic emergency form
- signed COVID activity liability release form
- signed OASD COVID acknowledgment form
Also, the appropriate athletic fee should be paid (if it has not been waived) before the first contest in each sport or payment arrangements must be made with the activities coordinator.
Please keep in mind that some sports conduct tryouts due to large numbers. Failure to be at tryouts can or will result in your child being unable to participate on a team.
Participation forms available on Infinite Campus Parent Portal
The district uses an electronic process for completing required athletics and activities paperwork (LEARN MORE). The required forms are be available via the IC Parent Portal and can be completed at any time for the upcoming school year. Forms must be completed before a student’s first day of practice/participation. Parents/guardians must complete the required forms for each participating high school student. Once a parent/guardian submits a form, the student will have access to sign the form via their IC Student Portal.

REMINDER: Hardcopies of the WIAA Physical Form and WIAA Alt Year Permit Card will continue to be required; these forms will NOT appear in IC as available to complete electronically.

WIAA Physical Forms, WIAA Alt Year Permit Cards, and Covid-19 Vaccination cards can be uploaded digitally via the google form links below, or provided to your child’s coach or athletic director.
