Rise Up
The OASD has partnered with Samaritan Counseling and Catalpa Health to establish site based services in the form of mental health screening and clinical mental health therapy in the school setting.
Catalpa Services
Catalpa Health Services:
Catalpa Health also provides on-site clinical therapy by licensed mental health providers to those students who are in need of support. Once a student is referred for these services, an intake meeting is scheduled with the student and his/ her parents. Signed parental consent is also needed in order to proceed. The program is currently in place at Washington Elementary School, Read Elementary School, Webster Stanley Middle School, North High School, and West High School.

Wellness Screen
Wellness Screening:
The Wellness Screen is an emotional/ mental health screening process provided by highly trained mental health professionals from Samaritan Counseling. The program is currently available to students in grades 9th and 11th at both North and West High Schools. Students who are identified as being in need of support through the screening process are referred to Rise Up or other community mental health providers.

Q: What is the Rise Up Screening Program?
A: The American Academy of Pediatrics (AAP) recommends an annual mental health screen for all school-age students. Mental health check-ups are just as important as annual physical checkups! The Wellness Screen Program uses a nationally recognized, evidenced based mental health screening tool to identify common emotional wellness concerns. Parents will receive information by letter and email in advance of the screening, explaining the process. Parents will be contacted by our team by phone as well as letter, if any concerns are identified in the screening for their student. When concerns are identified, our team provides robust ongoing support in helping families access further evaluation and resources. The Wellness Screen Program is voluntary, confidential, and FREE for families.
Q: What is Rise Up-Therapy Services?
A: Catalpa Health provides on-site mental therapy by a licensed mental health therapist whom specializes in working with children and adolescents. The Catalpa health therapist would be able to provide mental health therapy similar to what a client would receive at the clinic, onsite in the school.
Q: How can students access Rise-Up Therapy Services?
A: Families can access Rise-Up Therapy services by calling 920-750-7088, and requesting to schedule an initial appointment with a Rise-Up therapist. A school counselor or social worker at the identified schools are also able to complete a referral. If a referral is completed our scheduler calls families to initiate school based mental health services. At the initial appointment parents will be required to attend to sign paperwork to allow our mental health therapists to provide mental health counseling to your child. You will also be asked to provide insurance information, as our therapist’s do bill insurance companies for services similar to services in the clinic.
